Katy Trail Community Health is seeking a Medical Records Clerk who will be an essential member of the healthcare team. The medical records clerk works directly with each department within the organization to establish a smooth and successful transmission of patient documents, file maintenance, ROI authorizations and other essential clerical duties.

Responsibilities and Essential Functions:
Work cooperatively with Clinic providers and personnel to accomplish all goals and objectives of Katy Trail Community Health.
Coordinate the maintenance of the clinic's permanent medical records file system.
Coordinates file purging and archiving projects, assuring quality of archival images.
Coordinate permanent chart and loose sheet filing/scanning, monitoring compliance with accepted filing/scanning procedures to reduce misfiles and assuming responsibility for finding and fixing "lost" documents.
Assign and distribute all patient information, including but not limited to faxed, mailed, and printed documents, in an accurate and timely manner and as outlined in the distribution guideline document.
Mediate between the care team and the patient by monitoring the patient portal and the eprescription module continuously throughout each day.
Communicate with the receptionist, referral coordinator, and clinical staff to provide for efficient records flow through the system.
Handles all requests and inquires for patient health information whether received via mail, fax, phone, or in-person.
           o Verifies the patient identity and confirms that the authorization is valid
           o Ensures the requesting party has a legal right to request a patient's medical information.
Coordinate release of medical record information function: monitor turnaround time for release/request of information; monitor maintenance logs.
Provide excellent customer service by being attentive and respectful; insures understanding of customer request and follows-through as promised; and being proactive in identifying client concerns, or problems. Helps resolve customer service matters in a polite manner.
Maintain the records system in compliance with laws and regulations.
Assure patient confidentiality and adherence to HIPAA, HITECH, and OMNIBUS rulings
Maintain acceptable levels of loose files, incomplete records and uncompleted records requests, as agreed upon with supervisor.
Performs at established quantitative and qualitative work standards to meet departmental goals and objectives.
Presents a courteous and helpful demeanor, to all patients, visitors, and other staff members or any other person an employee encounters while representing Katy Trail Community Health
Promotes quality improvement, staff and patient safety, and culturally diversity through department operations and by personal performance.
Maintain current knowledge related to applicable statutes, regulations, guidelines and standards necessary to perform job duties.
Perform other duties as assigned.
Maintain an above average attendance record.
Prepare reports timely and accurately as requested.

Minimum Qualifications:
High school graduate or obtained GED; college coursework or significant medical records training preferred
One year experience in medical records preferred
Excellent attendance record
Demonstrate computer skills, word processing
Demonstrate ability to organize and categorize
Demonstrate well developed written and oral communication skills with ability to interact with providers and third parties, including ability to utilize tact and sensitivity to timing in personal transactions
Excellent Customer Service Skills with patients and coworkers, including but not limited to, exercising empathy exercising active listening skills, and meeting special needs in a timely manner
Ability to accurately record and transmit detailed information
Ability to comprehend, interpret, and effectively uphold the policy regarding release of information and patient confidentiality
Ability to exercise good judgment in evaluating situations and making decisions
Previous experience in operation of office machinery: Personal Computer and printers, copier, FAX, postage machine
Adequate vision. Ability to walk, bend, kneel and lift 25 lbs.
Assure patient confidentiality and adherence to release of information and other related clinic policies.
Ability and means to travel on a flexible schedule as needed, proof of liability and property damage insurance on vehicle used is required.
