Katy Trail Community Health is seeking a triage nurse that will develop an assessment of the patient's concern based on the conversation with the patient. In many instances, the triage nurse might deal with a wide spectrum of medical conditions. The nurse uses clinical judgment, knowledge and experience to direct the conversation with the patient and guide him/her in the decision to give self-care advice, refer the patient to primary care physician, make an appointment or instruct the patient to contact 911 for emergency assistance.

ESSENTIAL DUTIES & JOB RESPONSIBILITIES:

* Work cooperatively with clinic providers and staff to accomplish all goals and objectives of Katy Trail Community Health.
* Using clinical judgment, determine whether a patient needs care and if so, what kind of care.
* Exercise skill and care in giving advice, responding with appropriate standard of care for a specific case
* Documents both nurse advice and patient responses.
* Customer service skills: Answers calls from patients pleasantly and effectively; provides clear instructions for patients.
* Triages patient phone calls and patient nurse visits for minor health questions/ problems and refers questions/problems beyond scope of       practice to nurse practitioners, physician assistants, and physicians
* Maintains ability to follow through on requests made that fall under "Standing Orders" and the triage protocols.
* Handles calls referred from centralized scheduling. Triage nurse will follow practice guidelines and determine when a patient needs to be seen.
   All calls received will be returned by the triage nurse following emergency protocol guidelines but no later than 4 business hours.
* Coordinate Same Day Kids emails and requests.
* Provide education to patients, families, and other individuals providing care and/or services to the patient about managing their disease(s),    conditions, and supplemental information as requested and/or needed.
* Demonstrate the ability to communicate effectively with all members of the healthcare team, patients, and community representatives.
* Complete adequate medication reconciliation after hospital discharge via telephone or appointment.
* Maintains confidentiality of all patient information.
* Participates in Continuing Education as required by supervisor or state or federal guidelines.
* Ability to adapt to change as needed.
* Maintain excellent attendance record.

MINIMUM QUALIFICATIONS:


* Possess as a registered nurse in the state of Missouri.
* Possess current CPR certification.
* Possess basic keyboard skills and computer knowledge.
* Bilingual abilities encouraged..
* Must be able to see, hear, read, write, kneel, stand, sit and lift up to 10 lbs.
* Must be able to handle stressful situations.
* Possess knowledge of standard concepts, practices and procedures.
* Possess critical thinking and assessment skills.
* Maintain professional appearance and attitude at all times.
* Ability and means to travel on a flexible schedule as needed, proof of liability and property damage insurance on vehicle used is required.
* Excellent attendance record
* Must maintain patient confidentiality at all times.
